EoE Symptom Tracker We are delighted to provide you with our patient and carer symptom tracker for all those diagnosed with Eosinophilic Oesophagitis. This tool can be used prior to your routine hospital appointment, ready in preparation, or if you would like to track and log an increase in symptoms to report to your team. Why You Should Use The Symptom Tracker Stella, a member of our community, who was waiting for her diagnosis for 50 years, shares why you should track the symptoms. Watch this short video to hear her insights. "I would say: get all the information you can. There's a lot of information now on the EOS Network website, take that with you to your doctor.I would also recommend, keep a food diary for a few weeks and not just food, your symptoms as well. It gives you evidence. Most of the time, when you phone for an appointment you've been feeling bad, but it can be weeks before you actually see the doctor. By the time you see them, you're feeling better. Get as much evidence as you can, and be persistent. It will be hard... And don't be scared to show your emotions. There's the support group there on Facebook. And we're all in this together; we've been there, and we're coming through. We're keeping going; we're not giving up!" - Stella Read the full Stella's Story This patient tool works hand in hand with our food obstruction action plan, which can be found here Click Here to view and print your copy of this two-page tracker, which can be personalised online or left blank for paper/pen completion This resource was developed in collaboration with: the EOS patient/carer community our global professional network our medical advisory board With thanks to: Our volunteers, focus groups, survey participants and support from Dr Falk Pharma.
