The transition from paediatric to adult healthcare marks a vital phase for young individuals with chronic illnesses.

Often, patients become disconnected during this delicate transition, resulting in poor health outcomes, developmental setbacks, and lasting impacts on their independence and well-being.

To address this, the European Society of Paediatric Gastroenterology, Hepatology and Nutrition (ESPGHAN), together with the National Paediatric Gastroenterology, Hepatology and Nutrition (PGHAN) Societies, has initiated the Healthcare Transition Campaign.

Our CEO and Founder, Amanda Cordell, was pleased to participate in the development of this initiative as a part of the ESPGHAN Public Affairs Committee. 

This collective effort aims to increase awareness, encourage structured transition processes, and improve outcomes for young patients and their families.
